If you are running Internet Explorer and are getting prompted for a username and password to submit your form, please download and install Mozilla Firefox web browser.

First click on the green icon that says "Firefox 3.5 free download"

When it asks you:

1. Run   Save   Cancel - choose run

2. Run   Don't run - choose run

3. Next

4. leave as standard and uncheck "use Firefox as my default web browser"

5. Click next

6. Click "install"

7. Leave the check box checked and click "finish"

8. Once the firefox browser starts copy and paste this link into the address bar:

               Then click the little green arrow on the right side of the address bar, the online form should come up. Please try re-entering your form to complete your registration.
